The SAME FORM is now used for all GMP Staff memberships, whether it be a transfer or new application. If you are transfering you need to fill the form in with where you are transfering your sports & social payment TO and we will do the rest. We don't need to know where you are transfering FROM. Think of it as a brand new application.
